Configuration and Incident Data Collection

Customer Information
Customer Details, to be completed by Stellar.

Used for VM names, network hostnames, and internal references.

Primary point of contact for all technical matters.

Implementation Details
Specify information applicable to the entire IaaS implementation.

Select Migrate VMs if Stellar will migrate existing virtual machines. Select Build New VMs if new VMs will be built and data manually copied. Select Both for a combination.

Backup & Restore: Stellar uses Orion Backups toolchain. Manual Data Copy: Data/Apps manually installed on each VM.

Specify the approximate total of all data to be migrated.

Bandwidth available at the current data source (Mbps or Gbps).

Security Zones
List each security zone to be provisioned. Zones correspond to points of firewall control.

Networks
List each network to be provisioned. Select the security zone for firewall policy application.

Gateway
The gateway between your Orion services and the internet.

Most often, customers use the gateway as the literal "default gateway" for each of their cloud networks, as well as the termination point for VPN tunnels, external routing, and internet-exposed servers.

Select Orion NGFW for a fully managed Next Generation Firewall from Stellar & Palo Alto Networks.

Select the amount of bandwidth we should dedicate to your cloud services.

The method by which your remote offices will connect to your Orion services.

Remote Connectivity
Add each remote site that requires VPN connectivity to the Orion environment, including vendor tunnels.

VM Inventory
Manage the virtual machines to be provisioned in your Orion environment.

Portal Access
List each user that requires access to Stellar's Cloud Management Portal.

• Name: Provide the user's given first & last name.

• Email Address: Provide the user's email address. This will be the user's username when accessing the portal.

• Role: Select Admin for administrative privileges, or Read Only for view-only access.

Submission Security
This form requires a secondary authorization key to verify the sender's identity and prevent unauthorized API submissions.

Submission will be rejected if this key is missing or invalid.
